Job summary

Caringhand Home Health Solutions, Inc. is seeking a compassionate Registered Nurse for field-based, per diem home visits in California. You will assess patients, develop care plans, administer medications, educate families, and coordinate with physicians to support independent living at home.

Responsibilities include case management, discharge planning, supervising home health aides, and documenting care. Travel to patients' homes is required; this is not telehealth or work-from-home.

Qualifications

Must be a licensed RN in California with an active license.
Graduate of an accredited nursing program.
Bachelor's degree in nursing is preferred.

Responsibilities

Conduct initial patient and family assessments in the home.
Reassess nursing needs and changes in condition.
Develop and revise plans of care with patient goals.
Administer medications and treatments as prescribed.
Coordinate care with physicians and other healthcare professionals.
Educate patients and families about care plans and self-care.
Support discharge planning and case management as needed.
