Adaptive Home Health is seeking an RN Case Manager to deliver skilled home health care across Texas. You will assess patients, manage wounds and infusions, document OASIS, and coordinate with physicians and families to maintain an effective care plan.

This full-time, field-based role offers mileage reimbursement, a productivity-based pay model, and a mission-driven team focused on expanding access to high-quality home health care while supporting clinician autonomy.

Qualifications

• Current Texas RN license is required.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) certification required.
• Current Texas driver’s license and field-based work readiness.
• Strong time management in autonomous settings.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusion therapy.
• Build and maintain a plan of care through communication with physicians, care teams, patients, and families.
• Act as patient advocate ensuring continuity of care across disciplines.
• Drive case management tasks including coordination, follow-ups, and barrier resolution.
