Adaptive Home Health, LLC is seeking an RN Case Manager for full-time field-based work. You will own assessments, coordinate skilled visits, and manage OASIS documentation with a focus on wound care and infusion therapy. A productivity-based pay model offers weekly points with weekend flexibility as needed.

The role requires a Texas RN license, 3+ years of clinical nursing, CPR, and readiness to work in a field setting with strong communication to physicians, families, and the clinical team.

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) required.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians, care team, patients and families.
• Drive case management with care coordination, timely follow-ups, and barrier resolution.
