Job summary

Adaptive Home Health is seeking a Registered Nurse Case Manager for a full-time, field-based role across Texas. You will own patient assessments and skilled visits, manage wound care and infusion therapies, and coordinate with physicians, care teams, patients, and families to maintain a clear plan of care.

You will advocate for patients, ensure continuity across disciplines, and lead care coordination and follow-ups to drive positive clinical outcomes.

Qualifications

• Must hold Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) certification.
• Previous home health experience.
• Texas driver's license and field-based work availability.
• Strong time management in autonomous settings.
• Clear, professional communication with patients and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians, care teams, patients, and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, follow-ups, and barrier resolution.
