Adaptive Home Health, LLC is seeking an RN Case Manager to lead high‑acuity home visits across Texas. You will own patient assessments, document care, and drive detailed plans of care in collaboration with physicians, families, and the care team.

You will coordinate services, supervise relevant staff, and provide teaching while delivering compassionate, patient‑centered care in the home setting. This role offers a field-based schedule and a competitive productivity model.

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Experience with wound care and infusion therapies.
• CPR (BLS) certification.
• Previous home health experience.
• Current Texas driver's license and field-based mobility.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management: care coordination, timely follow-ups, barrier resolution.
• Coordinate services (PT/OT/ST/MSW/HHA) to execute the plan of care.
• Administer medications and provide teaching.
• Coordinate with clinical and operational teams for reliable, safe care.
• Supervise LVNs and HHAs as applicable.
• Participate in case conferences and in-services to align on quality and outcomes.
