Job summary

Angels of Care Pediatric Home Health in Savannah, GA is seeking a Registered Nurse Case Manager to lead a clinical team and manage a caseload of clients. You will train staff, onboard new hires, and collaborate with an interdisciplinary team to coordinate high-quality, patient-centered care.

The role includes field days for supervision and office days for administrative tasks, with regular local travel to support patient care and program operations.

Qualifications

• RN is required and licensed in Georgia.
• Degree from an accredited school of nursing.
• Home health experience preferred, with case management exposure.

Responsibilities

• Supervise and support direct care staff to ensure adherence to policies and regulations.
• Manage a caseload of clients, develop personalized care plans, and coordinate with external providers.
• Conduct comprehensive assessments and supervisory visits, documenting care plans per agency standards.
• Lead orientation, training, and competency verification for new hires.
• Act as liaison among staff, community, and healthcare providers for seamless communication.
• Provide in-field supervision and real-time guidance to clinicians.
• Participate in quality assurance, performance improvement, and staff development initiatives.
• Support on-call operations and ensure timely follow-up and accurate documentation.
