Qualifications

• RN license required and active.
• Degree from an accredited nursing program.
• Home health or pediatric experience preferred.

Responsibilities

• Supervise direct care staff and ensure policy compliance.
• Manage caseload and develop care plans.
• Conduct assessments and supervisory visits.
• Lead orientation, training, and competency verification.
• Coordinate with providers and community for seamless care.
