Comfort Keepers in Oklahoma City seeks an experienced Registered Nurse Case Manager for field-based, part-time/PRN/contract work. You will conduct client assessments, develop care plans, supervise caregiver activities, and ensure high-quality home care for seniors and veterans.

The role offers flexible scheduling, travel to client homes, and opportunities to earn in-home assessment and PDN compensation while supporting a compassionate team.

Formación

• Active and unrestricted RN license.
• Minimum of three years of experience in Home Health, Hospice, Rehabilitation, Home Care or PDN.
• Experience with geriatric populations and chronic conditions is preferred.
• Strong communication and interpersonal skills; ability to educate clients and families.

Responsabilidades

• Conduct intake calls and follow up with prospective clients and families.
• Perform initial in-home assessments and home safety evaluations.
• Develop, implement, and document individualized plans of care.
• Conduct periodic reassessments and update care plans as needed.
• Monitor client conditions and document changes appropriately.
• Coordinate care among clients, families, physicians, and staff.
• Lead supervisory visits to monitor service quality and caregiver performance.
