Comfort Keepers is seeking an experienced Registered Nurse (RN) Case Manager to join our team in Oklahoma City. This field-based role supports client assessments, care plan development, and ongoing supervision across home-based services.

We offer flexible scheduling as Part-Time, PRN, or Contract, with compensation for assessments, re-assessments, and documentation. Travel to client homes is required, and a valid RN license with 3+ years of relevant experience is preferred.

Qualifications

• Active and unrestricted RN license.
• Minimum 3 years in Home Health, Hospice, Rehab, Home Care, or Private Duty Nursing.
• Valid driver's license and reliable transportation.
• Ability to travel to client homes.
• Strong computer skills and electronic documentation.

Responsibilities

• Perform initial in-home assessments and safety evaluations.
• Develop, implement, and document individualized care plans.
• Conduct supervisory visits and caregiver coaching.
• Coordinate care among clients, families, physicians, and staff.
• Maintain accurate electronic client records in WellSky.
• Participate in QA/QI activities.
