Job summary

Comfort Keepers is seeking an experienced Registered Nurse (RN) Case Manager in Oklahoma to conduct client assessments, develop care plans, and supervise caregiver activities. This field-based role requires travel to client homes, flexible scheduling, and a commitment to delivering high-quality, compassionate home care for seniors and veterans.

Under supervision of the General Manager and Director of Clinical Services, the RN will support caregiver training, documentation, and care coordination,

Qualifications

• Active RN license required.
• Minimum 3 years in Home Health, Hospice, Rehabilitation, or Private Duty Nursing.
• Valid driver's license and reliable transportation.
• Ability to travel to client homes throughout the service area.
• Strong documentation and computer skills.

Responsibilities

• Perform intake calls and follow-up with prospective clients and families.
• Conduct initial in-home assessments and home safety evaluations.
• Develop, implement, and document individualized plans of care.
• Conduct periodic reassessments and update care plans as needed.
• Monitor client conditions and document changes.
• Coordinate care between clients, families, caregivers, physicians, and staff.
• Educate clients and families on health, wellness, medications, and fall prevention.
• Conduct supervisory visits to monitor service quality and caregiver performance.
• Maintain accurate electronic client records in WellSky and document visits promptly.
