Job summary
Safe Life Home Health Care is seeking a Home Health Registered Nurse Case Manager to join our Chicago team. The role emphasizes clinical assessment, patient-centered care, and coordination of home health services, with a focus on preventing hospitalizations and ensuring high-quality outcomes.
The ideal candidate has at least 1 year of home health experience, an Illinois RN license, CPR certification, and proficiency in WellSky Kinnser EMR.
Qualifications
- Valid Illinois RN license.
- Valid CPR Certification.
- Valid Driver's License.
- Valid Auto Insurance.
- Minimum 1 year home health experience.
- Proficiency in WellSky Kinnser EMR.
- Strong clinical assessment and communication skills.
- Compassionate and patient-centered approach.
Responsibilities
- Provide strong clinical skills and patient-centered care.
- Conduct comprehensive home health assessments.
- Develop and implement individualized care plans.
- Administer medications and treatments per orders.
- Monitor progress and document changes in condition.
- Educate patients and families on disease management and safety.
- Collaborate with interdisciplinary teams for care coordination.
- Maintain accurate medical records and comply with regulations.
- Respond to patient inquiries and participate in meetings.
