Job summary

Colorado Coalition for the Homeless in Denver is seeking a Nurse Case Manager to deliver community-based nursing care outside primary health centers. This field-based role collaborates with an integrated health and housing team to support individuals and families experiencing homelessness.

The position emphasizes trauma-informed, culturally aware care, with duties ranging from triage and care planning to medication administration and patient education.

Qualifications

• Associates, Bachelors, or Masters in Nursing from an accredited program.
• Current Colorado RN licensure.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide skilled nursing care per community standards.
• Conduct rapid and accurate triage assessments with care recommendations.
• Educate patients on diabetes, hypertension, nutrition, wound care, and prevention.
• Administer medications safely per protocols.
• Collaborate with a multidisciplinary team to coordinate care.
• Mentor new nurses and nursing students.
• Deliver patient-centered care within trauma-informed, harm-reduction model.
• Work day, evening, or weekend clinics as scheduled.
