Job summary

Adaptive Home Health, LLC is seeking a Registered Nurse (RN) Case Manager to deliver high-acuity home health care in a field-based role across Colorado. You will own assessments, manage skilled visits, and coordinate with physicians and families to ensure optimal patient outcomes.

The ideal candidate has a Colorado RN license, 3+ years of nursing experience, and proficiency with wound care and infusions. CPR/BLS, a valid driver's license, and strong time management are required.

Qualifications

• Must have Colorado RN license (unrestricted).
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) required.
• Current driver's license and field-based work readiness.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, focusing on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians, care team, patients and families.
