Affirmed Home Care, Inc. in Manhattan, NY is hiring a Full-Time Registered Nurse Care Manager to oversee a field-based caseload across Manhattan.

The role requires a NYS RN license, BSN (preferred), and 2+ years in clinical home care with advanced care skills including ventilator and tracheostomy care. You will assess clients, develop care plans, obtain physician orders, supervise field staff, and collaborate with families and physicians to ensure exceptional, client-centered care in the home

Qualifications

• Current New York State RN license in good standing.
• BSN preferred to support advanced care planning and supervision.
• Valid BLS certification required.
• Minimum 2 years of recent clinical home care experience.
• Hands-on clinical experience with advanced care (ventilator, tracheostomy, G-tube, IV infusions, wound care).

Responsibilities

• Conduct comprehensive client assessments and ensure highest quality of care.
• Develop, implement, and update individualized care plans.
• Obtain physician orders for home care services per NY regulations.
• Monitor clients and report changes to physicians and care team.
• Conduct supervisory visits and support field staff.
• Collaborate with clients, families, caregivers, physicians, and care team.
• Ensure care plans are followed and client needs addressed.
• Support client safety, independence, and quality of life at home.
• Maintain accurate clinical documentation.

Now Hiring: Full-Time Registered Nurse Care Manager

Are you an experienced Registered Nurse looking to take your career to the next level in a high-touch, concierge home care environment?

Affirmed Home Care is a leading private-pay home care agency dedicated to providing exceptional, personalized care to our clients. We are seeking a Registered Nurse Care Manager to join our growing team and oversee a caseload throughout Manhattan.

This is a 75% field-based position and is ideal for an experienced RN who is comfortable working independently, managing a caseload, supporting caregivers, and ensuring the highest standards of client care.

What You’ll Do

• Conduct comprehensive client assessments and ensure the highest quality of care


• Develop, implement, and update individualized care plans


• Obtain necessary physician orders for home care services in accordance with New York State regulations


• Monitor clients and report changes in their medical or mental condition to the attending physician and appropriate members of the care team


• Conduct supervisory visits and provide ongoing support to home care staff


• Collaborate with clients, families, caregivers, physicians, and the Agency’s care team


• Help ensure care plans are followed and client needs are appropriately addressed


• Support client safety, independence, and quality of life in the home environment


• Maintain accurate and timely clinical documentation


• Travel throughout Manhattan to conduct field visits and support clients and caregivers

What You Bring

• Current New York State RN license in good standing


• BSN preferred


• Valid BLS certification


• Minimum 2 years of recent clinical home care experience


• Hands-on clinical experience with advanced care, including:




• Ventilator management


• Tracheostomy care


• G-tube/PEG tube care


• IV infusions
