Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) to deliver skilled PTA visits in patients’ homes, under the supervision of a PT. This field-based role emphasizes gait training, transfers, strengthening, and home safety with clear communication to the PT and care team.

You will travel across the assigned Texas territory, carrying/transporting basic therapy equipment and following a structured productivity model to support patient progress and care coordination.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Comfort escalating changes to PT/RN/MD with clear documentation.
• Strong communication and documentation skills.
• Reliable follow-through on treatment plans and patient goals.
• Valid driver's license and willingness to travel daily across territory.
• Ability to carry/transport basic therapy equipment.
• Competency in core home health PTA scope: gait training, transfers, balance/fall prevention, etc.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, home safety, fall prevention.
• Set and track objective progress and communicate updates to the supervising PT.
• Coordinate with the care team and relay changes in condition.
• Coach patients and caregivers with practical home programs.
