Job summary

Adaptive Home Health, LLC is hiring a Physical Therapist Assistant (PTA) to deliver skilled PTA visits in patients’ homes across Texas. You will work under a supervising PT, focusing on functional mobility, home safety, and measurable outcomes.

The role includes a productivity-based compensation model and schedule flexibility to support field clinicians. Field-based work with home health care technology supports efficient documentation and visit coordination.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and comfort working independently in the field under PT supervision.
• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope: transfers, gait training, balance/fall prevention, etc.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, and home safety.
• Set and track objective progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination to align orders and keep the care team moving.
• Coach patients and caregivers with practical home programs that get implemented.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
