Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to manage in-home therapy plans, conduct comprehensive assessments, and deliver skilled visits across a field-based territory.

You will build a plan of care, track progress, and coordinate with the care team. The role offers a productivity-based earning model with target points per week and opportunities for bonuses. Drive and mobility equipment are supported.

Qualifications

• Active Colorado PT license in good standing; field-based practice.
• Strong clinical judgment and ability to work independently in the field.
• Clear documentation, strong communication, and timely follow-through.
• Current driver's license and willingness to travel daily across territory.
• Competency in core home health PT scope and PTA supervision as needed.

Responsibilities

• Own the caseload as a case manager with therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Create and execute a plan of care focusing on gait/balance, transfers, and mobility.
• Set measurable goals, track progress, and adjust treatment as needed.
• Coordinate with multidisciplinary team and align on orders and care.
• Supervise PTA when appropriate and coach patients/caregivers on home programs.
• Document promptly, thoroughly, and with defensible rationale.
