Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) for a field-based, full-time role across Texas. You will own the caseload as a case manager, perform in-home assessments, deliver skilled PT visits, and build a plan of care focused on gait, balance, transfers, and home safety.

You will work with a supportive clinical team and receive productivity-based pay (about $75 per productivity point) with opportunities to grow, plus schedule flexibility and access to Homecare Homebase (HCHB) and

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver's license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope (gait training, transfers, balance/fall prevention, home safety).

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, functional mobility, home safety.
• Set measurable goals, track progress, adjust treatment when needed.
• Drive multidisciplinary coordination with the care team.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
