Job summary

Qualifications

• Requires MS in Nursing.
• Requires current unrestricted NP or PA license in applicable state(s). DEA registration or eligibility required.
• CPR certification required.

Responsibilities

• Provides services in home settings to deliver care to patients.
• Develops and implements clinical plans of care for adults with chronic/complex conditions.
• Gathers history, performs exams, and develops treatment plans aligned with patient goals and conditions.
• Coordinates care with inter-disciplinary team and other providers to ensure timely resources.
• Educates patients and families about medication usage, adherence, and crisis prevention.
• Maintains medical records compliant with state regulations and company policies.
