Job summary

You will coordinate services, document progress, and collaborate with a multidisciplinary team to support housing and health outcomes.

Responsibilities include in-field client support, benefits linkages, documentation, and participating in trainings and supervision to ensure compliant, ethical care.

Qualifications

• Some college credits or 2 years of experience with homeless individuals.
• Ability to work with diverse populations in the field.
• Experience documenting in accordance with standards of care.

Responsibilities

• Conduct needs assessments and develop comprehensive treatment plans.
• Provide field-based case management and linkages to benefits and services.
• Document daily progress and collaborate with MDT; attend meetings and trainings.
• Coordinate transportation and maintain client records per HIPAA and CFR42.
