BAYADA Home Health Care is seeking an experienced Occupational Therapist for a full-time opportunity delivering home health visits in Fort Wayne, IN. Qualifications

• Current Indiana Occupational Therapist license.
• Prior home health experience preferred, but not required.
• Minimum one year of recent OT work experience.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status including muscle function, endurance, coordination, communication, and ADL/IADL performance.
• Evaluate cognitive, psychological and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards or barriers to more independent living.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Continually assess and revise the OT care plan in coordination with nursing and other team members.
• Educate patients and family members in rehabilitative care to promote health, safety and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.
