Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) to perform home health visits in the Fort Collins area. You will work with a multi-disciplinary team to provide rehabilitative care enabling clients to remain at home.

The role requires an active Colorado OT license, with at least one year of OT experience. Prior home care is preferred, strong communication and organizational skills are essential, and EMR experience is a plus. Mileage reimbursement is provided.

Qualifications

• Active Colorado OT license required.
• Minimum one year of OT experience; home care preferred but not required.
• Strong communication, organizational, and problem-solving abilities.

Responsibilities

• Make home visits to clients in designated Fort Collins area.
• Perform thorough evaluations of patients’ functional status and ADL/IADL skills.
• Develop individualized intervention plans to improve function and independence.
• Assess home environment for hazards and arrange adaptations if needed.
• Identify equipment needs and environmental modifications to enhance outcomes.
• Collaborate with nursing and multidisciplinary team on care plans.
• Educate patients and families on rehabilitative care for safety and independence.
• Document observations and interventions accurately using EMR.
