Job summary

Adaptive Home Health seeks an Occupational Therapist (OT) to perform in-home evaluations and skilled visits across Texas. You will design plans of care focused on ADLs/IADLs, safety, and independence, and coordinate with the care team to adapt when patients’ conditions change.

You’ll operate in a field-based setting with a productivity-based compensation model, supported by a team and technology to streamline charting and scheduling. Strong clinical judgment and independent work are essential.

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation skills.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
