Job summary

Alignment Health seeks a Field Remote Nurse Practitioner/Physician Assistant to join the Care Anywhere program in California. The role includes in-home assessments twice weekly, remote work three days a week, and strong collaboration with medical leadership to develop optimal patient care plans.

Qualifications

• Master’s degree from an accredited NP or PA program.
• Preferred: 1 year of clinical or home care experience; EMR experience preferred.
• Active NP or PA license for California with board certification; DEA, BLS, and current driver's license/insurance.

Responsibilities

• Conduct in-home assessments for Alignment members (2 days/week).
• Perform medication reviews and health screenings; plan care for 3 days/week from home.
• Provide patient education by aligning care desires with best care.
• Identify diagnoses to be assessed in care management and active medical management.
• Collaborate with Regional Medical Officer and team to develop appropriate care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead broader clinical team including nurses, health coaches, social workers, and coordinators.
