Job summary

sus.com is seeking a dedicated professional for field-based services to support homeless and formerly homeless individuals living with HIV/AIDS. You will implement comprehensive service plans, assist with housing, medical care, and transportation, and accompany clients to medical appointments.

Qualifications

• Minimum Associates Degree.
• Experience working with people living with AIDS, homeless people or substance users encouraged.
• Effective written and oral communication skills.

Responsibilities

• Direct, field-based services to enrolled consumers.
• Support the implementation of a comprehensive Service Plan for each enrolled consumer.
• Assist consumers with securing services for housing, medical care, nutrition and transportation.
• Accompany consumers to all primary care and specialty care appointments.
• Provide field-based Directly Observed Therapy (DOT) Services.
