Job summary

Housing Works in New York City seeks a Community-Based Prevention Navigator to engage clients where they are, offering prevention services outside health centers and linking to Housing Works' services.

You will work with a cross-functional team to identify and engage individuals seeking HIV testing, HIV prevention, and related services, guiding them through assessment, care planning, testing, and treatment as appropriate.

Qualifications

• Bachelor’s degree in a health field or equivalent in experience preferred.
• Minimum 1 year of relevant non-profit employment experience required (i.e. outreach, testing, group facilitation, case management, etc)
• Knowledge of HIV/AIDS, homelessness, substance use and mental illness, and LGBT youth issues
• Good problem-solving skills and ability to work autonomously
• Good verbal and written communications skills
