Job summary

Community Counseling Center is seeking a Community Support Caseworker to deliver community-based support through the CPRC program in Missouri. You will work in the community, including client homes or residential centers, and typically manage 20-25 clients with weekly meetings.

The role emphasizes a client-centered, strength-based approach. Strong communication and organizational skills, reliable transportation, and collaboration with the treatment team are essential to meet goals and connect

Qualifications

• Bachelor's Degree or four-year combination of education and qualifying experience.
• Must hold or obtain MO Class E Driver's License by start date, with proof of insurance.
• Travel in own vehicle is required.

Responsibilities

• Deliver community support services for assigned clients in the CPRC program.
• Manage a caseload of 20-25 clients and meet with them weekly.
• Document client interactions per agency guidelines by deadlines.
• Provide training and education to clients in daily living skills.
• Monitor treatment plan goals and maintain contact with the treatment team.
• Refer clients to appropriate community resources to improve functionality.
• Document according to clinical policies and program standards.
