Job summary

Staffing Solutions Organization LLC (SSO) is seeking a Medicaid Application Support Specialist for field-based work across New Hampshire. The role supports individuals and facilities with Medicaid LTC eligibility determinations and redeterminations, offering hands-on assistance and education to residents, families, providers, and administrators.

Travel to nursing facilities, hospitals, and designated sites is required, with a schedule of Monday–Friday, 8:00am–4:30pm ET.

Qualifications

• Bachelor's degree in social work, human services, business administration, or related field or equivalent experience.
• 1–3 years of experience in long‑term care, community health, case management or related settings.
• Ability to read, write, and speak English clearly.

Responsibilities

• Travel to assigned on‑site locations across New Hampshire to support nursing home staff and meet with residents and families for hands-on assistance.
• Review pending LTC applications for completion and compliance with state and federal requirements.
• Explain complex requirements to applicants and assist with completing applications during in-person or virtual appointments.
• Guide applicants through forms, gather documentation, and submit applications (electronic and physical).
• Educate applicants on Medicaid LTC rules, including spend down, asset transfers, spousal impoverishment, and third‑party liability.
• Review submitted documents, identify missing verifications, and help applicants gather missing items.
• Provide updates to the Eligibility Supervisor and Project Management team on assistance provided and barriers encountered.
