Job summary

UPMC Health Plan is seeking Service Coordinators to join the Community HealthChoices team serving Blair and Cambria Counties. This hybrid position requires daily community travel to assess and coordinate LTSS, medical, behavioral, and social needs for seniors and individuals with disabilities.

The ideal candidate will have strong case management experience, familiarity with in-home assessments, and the ability to work with adult populations facing complex health needs.

Qualifications

• Bachelor's degree in social work or related field or three years of social service/healthcare experience.
• Experience with case management, care coordination, and home visits preferred.
• Strong ability to work with adults with complex medical, behavioral, and social needs.

Responsibilities

• Establish relationships with internal teams and partner providers to coordinate medical, behavioral, and social services.
• Manage an active caseload and track participant progress for regulatory reporting deadlines.
• Lead care planning processes and assist Members in obtaining LTSS and Medical Assistance services.
• Coordinate access to LTSS, medical services, and community resources to support independent living.
