Job summary

UPMC Health Plan in Lackawanna County seeks Service Coordinators to join the Community HealthChoices team. This hybrid role requires daily community visits to assess and coordinate LTSS, MA, and related services for adults with complex needs.

Qualifications

• Must have a Bachelor's degree in social work, psychology, or a related field.
• Three years of experience in a social service or healthcare setting is required.
• Strong background in case management and care coordination.
• Experience performing in-home assessments and visits with adults.
• Compassionate, resourceful, and able to work independently in the community.

Responsibilities

• Educate and coordinate resources; medical, behavioral, and social services for Members.
• Manage an active caseload with state ratios and document progress monthly.
• Prepare reports and update service plans; ensure timely comprehensive assessments.
• Lead PCSP process and assist Members in obtaining LTSS and HCBS.
• Coordinate access to LTSS, MA, and other supports; explain options and rights.
• Field-based work about 75% of the time; virtual work as needed.
