Qualifications

• 2 years’ experience in behavioral health or related field.
• Must reside in Newport News or Hampton, Virginia
• Must travel up to 75% of the time. Mileage reimbursed per policy

Responsibilities

• Collaborate to facilitate care management to achieve appropriate health outcomes.
• Evaluate member needs and recommend approaches to case resolution.
• Coordinate care plan activities and monitor progress.
• Identify risk factors and escalate quality of care issues.
• Provide coaching and support to empower independent health decisions.
• Assist members in engaging with providers in decision-making.
