Job summary

Centene Corporation seeks a Behavioral Health Case Manager to develop and coordinate care plans for members with significant mental health needs. The role requires licensure and 2–4 years of related experience, with travel to visits across the service area in Texas.

You will educate members and families on benefits and services, liaise with physicians and facilities, and document care plans in the electronic system while ensuring regulatory compliance.

Qualifications

• Master's degree in Mental Health or Social Work or nursing with behavioral health experience.
• 2–4 years of related case management or care coordination experience.
• Licensure as LCSW, LPC, LMHC, LMFT, or RN with behavioral health experience may be required.

Responsibilities

• Evaluate complex high-risk members and coordinate a plan of care.
• Act as liaison between member, family, physician, and facilities.
• Provide education to members and caregivers on services and benefits.
• Perform home or site visits to assess needs and coordinate resources.
• Ensure compliance with clinical guidelines and state/federal regulations.
