Job summary

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) in Eastern Ohio to manage a designated caseload, conducting assessments and developing Person Centered Plans in collaboration with the care team.

This field-based role requires a BA/BS and at least 2 years in social work, with travel to client sites and occasional office meetings; hybrid options may be available per policy.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Perform face-to-face program assessments and identify member needs using predefined questions and motivational interviewing techniques.
• Coordinate member cases with the clinical team to provide comprehensive care management and support.
• Manage non-clinical needs of members to ensure cost-effective utilization of long-term services and supports.
• Document short and long-term goals with the member and care team, including potential alternative levels of service.
