Ennoble Care in Little Rock, AR seeks an LPN Team Lead to oversee 8–12 Embedded Care Coordinators across partner clinics. This senior, field-based role blends direct patient care duties with leadership and program management within our Care Coordination program.

Qualifications

• CMA or LPN license required.
• Valid driver\'s license and access to a vehicle required.
• At least 2 years of management experience in a healthcare setting.
• Knowledge of customer needs assessment, meeting quality standards, and evaluating customer satisfaction.
• Knowledge of business and management principles for strategic planning and resource allocation.
• Proficiency in English language, spelling, and grammar.
• Understanding of curriculum and training design and measurement of training effects.
• Familiarity with personnel management principles and procedures.

Responsibilities

• Maintain embedded presence at assigned clinics and supervise a team of Embedded CCs.
• Execute core CC duties: prescription management, orders, referrals, check-ins, follow-ups.
• Serve as the bridge between clinic staff and Ennoble Care\'s services.
• Conduct proactive rounds and respond to acute patient needs.
• Meet daily task goals (400 mins) unless in training.
• Support telehealth and RPM programs within facilities.
• Oversee 8–12 Embedded CCs across facilities/markets.
• Provide day-to-day coaching and ensure standard adherence.
• Escalate issues and support problem-solving for the CC team.
• Monitor KPIs and address performance gaps.
• Coordinate with providers to ensure needs are met.
• Partner with leadership on staffing and development.
• Model documentation quality and patient engagement.
• Backfill CCs as needed for continuity.
• Travel to facilities to oversee coordinators.
