Job summary

Life Care Centers of America is seeking an LPN Unit Care Coordinator to supervise and coordinate patient care on the unit, collaborating with nurses, CNAs, and families to uphold high standards of care.

You will direct daily nursing operations, document care, administer medications as ordered, and respond to emergencies while ensuring compliance with regulations and Life Care standards. Requires an accredited LPN program and active licensure, CPR, and preferred 1 year geriatric experience.

Qualifications

• Active LPN/LVN licensure in the state of employment.
• CPR certification required or obtained during orientation.
• Minimum of 1 year geriatric nursing preferred.

Responsibilities

• Direct daily functions of unit nurses and CNAs to provide floor leadership.
• Chart appropriately, accurately, and timely.
• Provide and coordinate patient care to maintain patient well-being.
• Accurately prepare and administer medications as ordered.
• Respond in leadership capacity to emergencies affecting patient/staff safety.
• Coordinate care plans with the RN.
• Demonstrate customer service and a positive attitude toward patients.
