Job summary

Community Bridges, Inc. is seeking an Independent Living Specialist to support individuals with serious mental illness in their homes and in the field.

You will help clients develop independent living skills such as budgeting, transportation, hygiene, and community supports, and provide after-hours coverage as needed. The role emphasizes field work, collaboration with clinicians, and maintaining engagement with clients to sustain recovery, while meeting credential and background requirements

Qualifications

• Experience working with the SMI population across settings and levels of care.
• Prior outreach and engagement activities in behavioral health preferred.
• Clear background requirements including driving and fingerprint clearance.

Responsibilities

• Work with SMI clients in homes or in the field to build independent living skills.
• Field work up to 80% of time; provide after-hours and on-call supports.
• Assist clients in daily living tasks and community-based supports.
