Responsibilities

• Perform head-to-toe assessments and provide symptom management for home-based patients with terminal illnesses.
• Supervise LPNs/CNAs, monitor medications, assess DME needs and provide wound care.
• Offer emotional support and education to patients and families facing death and dying.
