Job summary

Preferred Hospice of Missouri is seeking a dedicated RN Case Manager to serve patients in the Farmington, MO area. The role is primarily field-based, visiting patients in homes, facilities, and other settings within a defined geographical area.

You will perform head-to-toe assessments, manage symptoms, supervise LPNs/CNAs, and provide education and emotional support to patients and families facing terminal illness. Office visits occur occasionally, and full-time benefits are offered.

Qualifications

• The successful candidate must be currently licensed in the State of Missouri as an RN and live within a commutable distance to the office.
• Willing to train the right candidate. A gentle, loving heart is a must. Must be able to provide love and support to patients and families facing a terminal illness.
• Computer and smart phone skills for EMR charting, email, mileage apps.
• Office hours Monday through Friday, 8am to 5pm. Reliable transportation and car insurance required.

Responsibilities

• Hospice RN Case Managers see patients in private homes, assisted living facilities, skilled nursing facilities, and anywhere a patient calls home.
• The RN Case Manager performs a head-to-toe assessment, provides symptom management and skilled care to patients facing terminal illnesses as well as emotional support and education on death and dying.
• The RN Case Manager supervises LPNs and CNAs, monitors medication lists and appropriateness of medications, assesses for DME needs, provides wound care and other treatments as needed.
