Job summary

Regional Hospice is seeking a Full Time RN Case Manager to serve Lebanon, MO and surrounding areas. The role involves driving between patient homes, clinics, and facilities with autonomy and teamwork in a dedicated hospice environment.

You will conduct assessments, manage symptoms, and educate families facing terminal illness while supervising LPNs/CNAs and coordinating care with the office as needed.

Qualifications

• Must be currently licensed as an RN in Missouri.
• Able to chart and communicate using EMR and apps for mileage computation.
• Strong teamwork and compassionate care for patients and families.

Responsibilities

• See patients in private homes, assisted living facilities, skilled nursing facilities, or anywhere the patient calls home.
• Perform head-to-toe assessment, manage symptoms, provide education on death and dying, and offer emotional support.

We are a well-established hospice company looking to add to our growing team!! Regional Hospice has 3 offices across Southwest Missouri.

As of 8/15/2026 there is a $10,000 Sign On Bonus for Full Time. It is paid in 4 increments with no obligation to pay it back.

We are currently looking for a Full Time RN Case Manager to serve patients in the Lebanon, MO area. We can do part time or PRN if needed.

The hospice nurse spends most of their work day in their car driving to see patients within a set geographical area. Some required meetings do occur at the office on occasion. This job affords a lot of autonomy as well as great team work.

DUTIES

• Hospice RN Case Managers see patients in private homes, assisted living facilities, skilled nursing facilities, and anywhere a patient calls home.
• The RN Case Manager performs a head-to-toe assessment, provides symptom management and skilled care to patients facing terminal illnesses, provides emotional support and education on death and dying.
• The RN Case Manager supervises LPNs and CNAs, monitors medication lists and appropriateness of said mediations, assesses for DME needs, provides wound care and other treatments as needed.

QUALIFICATIONS

• The successful candidate must be currently licensed in the State of Missouri as an RN and live with in a commutable distance to the office.
• Willing to train the right candidate. A gentle, loving heart is a must. Must be able to provide love and support to patients and families facing a terminal illness. Must be able to work as a team player and represent our company well in the community.
• Computer and smart phone skills for EMR charting, email, apps for mileage computation, etc.
• The offices are open Monday through Friday, 8am to 5pm. Reliable transportation and car insurance required.
