Job summary

Reliant at Home is seeking a Hospice RN Case Manager to cover Fort Worth and surrounding areas. The role requires hospice experience and RN licensure, with Home Care Home Base EMR experience a plus.

Responsibilities include comprehensive patient assessments, care planning, family education, on-call duties, and ensuring quality, compliant care across the care continuum. Fort Worth area knowledge is preferred.

Qualifications

• Graduate of an accredited nursing program (Diploma/ASN/BSN).
• Current RN license and Driver’s License.
• Two years’ RN experience; hospice preferred.
• CPR certification required.
• Must read, write and comprehend English.

Responsibilities

• Initial and ongoing comprehensive assessments of disease impact on functional needs.
• Apply admission criteria to hospice care and establish level of care.
• Develop and revise the plan of care to ensure quality continuity.
• Educate patient and family about disease, end-of-life care and self-care.
• Determine service scope based on acuity; assess caregiver capacity.
• Provide clinical direction to Hospice Aide and LPN/LVN.
• Prepare progress notes showing progress toward goals.
• Inform physician and team of changes and outcomes; monitor care.
• Ensure compliance with third-party payor requirements.
• Perform on-call duties; provide on-call service as assigned.
• Communicate effectively with patient, family, and health team.
