Job summary

Reliant at Home seeks a Full-Time Hospice RN to cover Fort Worth and surrounding areas in a home health setting. You will perform assessments, plan and adjust care, educate patients and families, and collaborate with the IDG to ensure quality end-of-life care.

Qualifications

• Current RN license for Texas
• Two years nursing experience preferred, hospice preferred
• Strong end-of-life care skills and assessment ability

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs
• Develop, implement, and revise plan of care for hospice patients
• Educate patient and family on disease process, self-care, end-of-life care; coordinate with IDG
• Provide on-call support and effective communication with care teams
• Ensure accurate documentation and progress notes for care milestones
