Reliant at Home seeks a PRN Hospice RN Case Manager to cover Fort Worth and surrounding areas. The role focuses on comprehensive assessments, care planning, and family education within hospice care. Hospice experience is preferred, and Home Base EMR experience is a plus.

Qualified applicants will hold an active RN license, CPR certification, and reliable transportation. Two years of RN experience in a clinical setting is desired, with a preference for hospice background.

Qualifications

• Graduate of an accredited nursing program.

• Current RN license in Texas or applicable Compact.
• Two years RN experience in clinical care; hospice preferred.
• CPR certification required; English literacy required.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Conduct initial and ongoing assessments of patient needs in hospice care.
• Determine admission and re-certification eligibility for hospice services.
• Develop and update care plans to ensure quality and continuity of care.
• Educate patients and families about disease process, end-of-life care, and self-care.
