Job summary

Elevation Hospice of Colorado is seeking a Community Liaison to promote hospice programs and services within a designated territory. This field-based role involves building relationships with physicians, hospitals, discharge planners, and other referral sources to enroll patients in hospice in a timely manner.

The Community Liaison will generate referrals, communicate with partners, and develop marketing plans designed to meet budget projections while maintaining confidentiality and compliance

Qualifications

• Bachelor’s degree preferred; other experience may substitute.
• 1–2 years of hospice/palliative marketing experience preferred.
• Position is field based and requires 80% travel within assigned geographic area.
• Ability to market collaboratively with customers, referral sources, and the community.
• Excellent written and verbal communication; computer literate.

Responsibilities

• Generate patient referrals through new and existing sources.
• Maintain regular communication with referral sources to optimize hospice service delivery.
• Identify opportunities for expanded services to meet community needs.
• Build and maintain relations to improve understanding of hospice programs.
• Develop and implement marketing plans to meet budget projections.
• Represent the company in community events and meetings.
• Participate in team meetings and other related activities as needed.
• Comply with Medicare, Medicaid, HIPAA regulations and internal policies.
