Job summary

Mishawaka South Bend Hospice is seeking a Registered Nurse Case Manager to join our growing team. You will support hospice patients at home or in facilities, delivering skilled nursing care and end-of-life support in partnership with the interdisciplinary team.

The role emphasizes compassionate care, independent field work, and collaboration with physicians, caregivers, and colleagues to ensure quality outcomes and adherence to hospice standards.

Qualifications

• Active RN license.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Ability to work independently in the field.
• Strong communication and documentation skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
• Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase).
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs as part of an on-call rotation when applicable.

Overview

The Care Team is seeking a Registered Nurse Case Manager to join our growing team! The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Territory: Up to a 1 hour radius of Mishawaka, IN
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only
