Job summary

UT Southwestern Medical Center in Dallas seeks a Home Health RN - Field Case Manager (PRN) to perform comprehensive nursing care per the client's plan of care and coordinate services in the home health setting.

The role requires an accredited nursing program graduate with 2 years RN experience, including a year in adult IV therapy; Texas RN license and BLS. A BSN is preferred. Excellent communication, assessment, and documentation skills are essential.

Qualifications

• Graduate of an accredited nursing program.
• Two years of RN experience with one year in adult IV therapy.
• Active RN license in Texas or a BNE Compact State; valid driver's license and BLS certification.

Responsibilities

• Provide direct nursing care and observe patients per plan of care.
• Conduct initial and ongoing home health assessments (including OASIS as applicable).
• Maintain accurate patient records, med profiles, care plans, and notes.
