UT Southwestern Medical Center in Dallas is seeking a Home Health RN Field Case Manager (PRN) to deliver comprehensive nursing care under the client’s plan of care. You will perform OASIS-like assessments, coordinate services, and provide teaching and training to patients and families.

The role requires active RN licensure in Texas (or BNE Compact State), two years of RN experience with one year in adult IV therapy, and BLS certification.

Qualifications

• Graduate of accredited nursing program.
• Two years of RN experience with one year of adult IV expertise required.
• RN license in Texas or BNE Compact State; valid Texas driver's license and proof of insurance.
• BLS certification by the American Heart Association (AHA) or American Red Cross (ARC) upon hire.

Responsabilités

• Provide direct nursing care and conduct initial and ongoing home health assessments.
• Evaluate outcomes, plan discharge needs, and coordinate with other disciplines.
• Maintain patient records, med profiles, and care plans.
• Instruct patients and families on product use and IV therapies.
• Administer meds and monitor IV fluids as prescribed.
• Perform phlebotomy and point-of-care testing.
• Participate in CQI and supervise Home Health Aides and LVNs as required.
• Participate in on-call rotation and emergency coverage per policy.
