Job summary

MDS Staffing is seeking an experienced Home Health RN Case Manager for a field-based role serving the Dallas/Fort Worth area in Texas. You will manage an assigned patient caseload, perform skilled nursing visits, and coordinate plans of care with physicians and families in a Medicare-certified home health setting.

The role requires an active Texas RN license and recent home health case management experience, with strong assessment, documentation, and time-management skills.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong knowledge of Medicare home health, OASIS, and plan-of-care management.
• Ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, recertifications, and discharges.
• Develop and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Provide education to patients and caregivers on disease management and plan of care.
• Coordinate care with therapy, social work, and office leadership.
• Maintain compliance with home health regulatory standards.
