Paragon Behavioral Health Connections seeks a Bachelor-Level Care Manager to provide intensive, community-based care management within the CAP Program, with a focus on Mercy Housing residents in the Denver Metro area. The role involves extensive fieldwork, outreach, and collaboration with housing partners and providers to support housing stability and behavioral health goals.

Responsibilities include coordinating services across health, housing, and social supports, assisting with treatment

Qualifications

• Bachelor's degree in social work, psychology, or related behavioral health field.
• Minimum one year of experience in case management or related human services.
• Valid driver's license and reliable transportation required.

Responsibilities

• Provide intensive, community-based care management in clients' homes and communities.
• Coordinate services across behavioral health, medical, housing, employment, and social supports.
• Assist clients with identifying goals, barriers, and progress toward recovery.
• Support housing stability and connections to community resources.
• Maintain HIPAA-compliant documentation and timely service notes.
• Travel to Mercy Housing properties and various community sites for on-site support.
