Job summary

Urban Alchemy is seeking a Manager, High Acuity Care Coordination in Portland to mentor the Care Coordinators in trauma-informed engagement and de-escalation. You will manage the most complex cases, design stabilization plans, and coordinate with medical, mental health, and housing partners to ensure guest-centered care.

Qualifications

• 3+ years in social services, street outreach, or intensive case management involving high-acuity populations.
• Experience with CoC, local providers, and referral pathways.
• Demonstrated TIC, harm reduction, and de-escalation skills.

Responsibilities

• Provide on-site mentorship to Care Coordinators, modeling de-escalation and trauma-informed communication.
• Lead intensive case navigation for complex high-acuity cases and create stabilization plans.
• Facilitate case conferences with internal and external partners to align on goals and care plans.
• Advocate for equitable guest access and navigate system barriers with external providers.
• Oversee high-acuity case documentation and ensure quality and grant compliance.
• Coach team to strengthen client-centered care and crisis intervention skills.
• Lead post-incident debriefs to support staff wellness and resilience.
