Job summary

Bluestone seeks a Geriatric Care Manager to support chronically ill patients in senior living communities. This field-based role links patients, families, and providers to execute unified care plans and optimize outcomes.

Expect 90% field work across Buffalo/Monticello/Elk River with 8:00–17:00 hours Monday-Thursday and 8:00–15:00 Friday; full-time, no evenings or weekends. Salary ranges from $68,500 to $80,000 USD per year.

Qualifications

• Bachelor's degree or higher; licensed personnel preferred - LPN, RN, or Social Worker.
• 3–5 years of experience in value-based care, population health, case management, care coordination and/or discharge planning.
• Experience in behavioral health preferred.

Responsibilities

• Develop and manage individualized care plans and support honest advance care planning discussions with patients and families.
• Collaborate with MDs, CNPs, and PAs to provide real-time observations and adjust care strategies.
• Lead transitions from hospital/rehab to home to prevent readmissions and manage 30-day outcomes.
